Incident E-Report Form 

This is an E-Complaint Form provided by the Orfordville Police Department for the convenience of Orfordville Citizens.  Often our citizens fail to call and make us aware of incidents which we should be aware of, for whatever reason.  We are able to serve you better and provide security to our citizens if we are informed of what is occurring within our Community.  However minor the incident seems to be, please at least make us aware of it so we can take an appropriate action, even if that action is only being aware. Should you not wish contact, we can assure you your information will remain confidential, and is used solely for Law Enforcement Purpose:
